Job Title: Physical Therapist Profession: PT Specialty: Therapy Duration: 13 weeks Shift: Days Hours per Shift: 8:00 AM to 5:00 PM Experience: Minimum years of experience not specified License: Current Physical Therapist Licensure Certifications: BLS certification via AHA required Must-Have: - Bachelor's, Master's, or entry-level Doctorate of Physical Therapy degree from an accredited college or university - Licensed as a Physical Therapist - BLS certification Description: Provides Physical Therapy services to referred patients, including assessment, treatment plan development, and implementation with follow-up and discharge planning. Actively assists with clinical aspects of the department, including program development, department operations, and performance improvement activities. Supervises and coordinates student programs and oversees Licensed Physical Therapy Assistants, Rehabilitation Technicians, and volunteers. Performs comprehensive diagnostic evaluations to determine the physical and cognitive abilities of inpatients and outpatients, considering diagnosis and age-specific needs. Develops and implements individualized treatment plans with patient and family participation, considering diagnosis and age-specific needs. Adheres to documentation policies and procedures, including Initial Patient Evaluations, daily and weekly progress notes, discharge summaries, and other appropriate notes. Participates in patient care conferences, family conferences, in-service programs, and departmental and interdepartmental meetings. Counsels patients, families, and other involved individuals regarding evaluations, care, and treatment. Initiates and completes discharge planning in a timely manner, providing effective education and training. Assists with the coordination and supervision of student programs as well as the volunteer program. Actively contributes to departmental initiatives, including orientation of new associates and scheduling of patients to maintain quality patient coverage. Demonstrates adherence to core values and participates in performance improvement activities. Maintains departmental productivity standards and identifies educational opportunities for self and the community. Participates in research activities as appropriate. Demonstrates competence in performing assigned patient care responsibilities that meet the age-specific and developmental needs of patients. Adapts assessment and treatment methods to accommodate the unique physical, psychological, cultural, and age-specific needs of each patient. Fosters positive relationships with members of the medical staff and referral sources. Performs other related duties as directed.
